The Nurse Monitoring Program provides nurse monitoring services to individuals receiving personal care assistance and other services from home and community based programs funded by the Maryland Department of Health.
Nursing assessments are completed by a Community Health Nurse in the client’s home. The comprehensive assessment includes medical, nursing, functional, psycho-social and environmental components. The assessment is also used to determine the frequency of nurse monitoring visits.
The Nurse Monitors work closely with the Support Planning Agencies, family members and clinicians to address the needs of the client.
There is no cost to the client for Nurse Monitoring Services.
Prince George's County residents who are receiving personal assistance services from Community First Choice (CFC), Community Personal Assistance Services (CPAS), Increased Community Services (ICS), and Community Options Waiver Programs.
