To receive information for Ambulance Reports, please mail your request to:

All requests for reports must be postal mailed to the address above.  No walk-ins, No Fax, No e-mail will be accepted as a form of request.

Procedures to request an ambulance report

Letter of request on company letterhead for businesses or a full sheet of paper
  • $5.00 check or money order payable to Prince George’s County (Does not apply for subpoenas and This fee is per incident/not per patient)
  • Patient Name. Please indicate if transported as a John/Jane Doe
  • Date of Incident
  • Approximate time is acceptable
  • Location of Incident. Include main street & cross street if it’s an accident
  • Signed Medical Release Authorization HIPPA – Does not apply if you are the patient. Attorneys/Insurance Companies or if a request is being made on behalf of the patient. Does not apply to minor children
  • Power of Attorney (If person is living, but cannot request for themselves)
  • Letter of Administration or Executor documentation (If person is deceased)
  • Self-addressed - stamped envelope
