The Primary Healthcare Strategic Plan reflects the ongoing commitment of Prince George’s County to improving the healthcare system and the health of its residents. The plan focuses both on expanding access to patient-centered primary care that is part of an integrated care system and on building a culture of health within the county. In order to achieve the triple aim (improving health outcomes, reducing per capita costs, and improving patient experience) it will be necessary to improve coordination of care within and across systems.
The county must also continue to address the conditions that shape health in the environments where people live, work, and play. The Primary Healthcare Strategic Plan fully embraces these concepts of population health and is on the leading edge of healthcare reform in the United States. In collaboration with all partners and stakeholders, the Primary Healthcare Strategic Plan will reach its goals of expanding access and improving residents’ health while contributing to the county’s economic development.
Prince Georgefs County, Maryland is poised for changes that will lead to improved health and quality of life for its citizens. Plans for a transformed new regional health care system that focuses on population health are under way through a unique partnership among the County, the state and academic and health care institutions. These plans come at a time of great momentum at the national, state and County levels to advance health care reform and eliminate health disparities.
