Community Health Worker Initiative

Community Health Worker Initiative

The Community Health Worker Initiative offers Prince George’s County residents free access to a Community Health Worker (CHW) who will help connect them with the health and social services they need. Through Community Health Workers, we aim to identify the needs of our residents and increase their access to quality health care.

What is a Community Health Worker?

A Community Health Worker is a front-line public health worker who works with residents to help them access the health and social services that they need. A Community Health Worker (CHW) can provide the following services to the community:

  • Help residents set and achieve personal health goals and adopt healthy behaviors.
  • Refer residents to medical transportation and other types of assistance.
  • Provide social support and refer clients to professional counseling.
  • Guide residents toward Health Education, including self-management of disease screenings, prevention, and management of chronic health diseases.
  • Coordinate resident care and case management.
