How to Get a Maryland Death Certificate

Requests for certified Maryland death certificates may be made at the Hyattsville location for deaths that occurred in Maryland after January 1, 2015.

Easily obtain your vital records from Maryland Vital Records. Maryland Vital Records offers the convenience of ordering birth and death certificates online through VitalChek. You can easily and securely order your vital records online in a few simple steps.

Who Can Request a Copy

You may obtain a Maryland Death Certificate if you are:

  • A funeral director
  • A surviving relative of the deceased  
  • A beneficiary of the deceased
  • An authorized representative of the deceased

Fee

There is a $25 fee for the first certified death certificate and $20 for each additional certificate purchased on the same day. Cash, check, money orders, and credit or debit cards (Visa, Discover, MasterCard) are accepted as payment.

  • A $2.50 service fee applies to credit/debit card transactions.
  • Check or money order should be made payable to Prince George’s County Health Department
