Complete the form, when you are finished print it (you may want to print an additional a copy to keep in your records). Include a copy of your valid government issued photo ID and a self-addressed stamped envelope. Mail your form and a copy of valid government issued photo ID to: Police Records, 4923 43rd Avenue Hyattsville, Maryland 20781 Incident reports, and Accident report are $10.00 per copy. Accident Photographs are $20 per roll. Payment options are money orders, and business checks. Make your payment to Prince George’s County, Maryland. Personal checks not accepted.
