Requests for certified Maryland death certificates may be made at the Hyattsville location on a walk-in basis only for deaths that occurred in Maryland after January 1, 2015.

Certificates for deaths that occurred prior to January 1, 2015 can be obtained from the Division of Vital Records, 6764-B Reistertown Road Baltimore, MD 21215.

Who Can Request a Copy

You may obtain a Maryland Death Certificate if you are:

• A funeral director
• An immediate relative of the deceased
• A beneficiary of the deceased
• A person of tangible interest

Fee

There is a $25 fee for the first certified death certificate and $20 for each additional certificate purchased the same day. Cash, check, money orders, credit or debit cards (Visa, Discover, MasterCard) are accepted as payment.

• A $2.50 service fee applies to credit/debit card transactions.
