The Immunizations Program provides routine vaccinations against childhood diseases to children and young adults, ages two (2) months to 18 years, with limited or no health insurance year-round. Immunization outreach and education are provided to families with children under the age of four who are not up-to-date on required immunizations.

Clients

Prince George’s County provides immunizations to residents:

  • Covered by or enrolled in Medicaid or medical assistance programs;
  • Who are 2 months to 18 years old and are uninsured or under-insured;
  • Who are Native American (American Indian) or Alaskan Native;
  • Who have health insurance that does not cover (pay for) vaccines

Fees

Fees are based on a sliding scale.

•    Parent's ID
•    Child’s ID (Birth Certificate or Passport)
•    Child’s Entire Immunization Record
•    Medical Assistance Card or Managed Care Organization (MCO) Insurance Card
•    Proof of Residency
•    Proof of Income (If your child does not have insurance)

DOCUMENTOS IMPORTANTES PARA TRAER:
•    Identificacion de Padre/ Guardian legal
•    Identificación del niño (Certificado de nacimiento o Pasaporte)
•    Registro completo de inmunización del niño
•    Tarjeta de seguro médico 
•    Prueba de residencia
•    Comprobante de ingresos (si su hijo no tiene seguro)
