System of Care (SOC) Service Providers
In the System of Care, our target population is Prince George’s County child/youth residents aged 0 to 21. These individuals may have or be at risk of developing serious emotional disturbances and may be at risk of entering the foster care or juvenile justice system.

Healthy Transitions Service Providers
The Healthy Transitions program is designed for Prince George’s County young adult residents aged 16 to 25. This program caters to individuals diagnosed with or at risk of developing severe mental health conditions. Specifically, it supports those entering or aging out of foster care, as well as those entering or transitioning out of the juvenile justice care and custody system. We provide a range of essential services through our providers.

Referral Process
To access our services, the referral process begins with various sources, including the Department of Social Services, Department of Juvenile Services, Prince George's County Public Schools, or through self-referral. Once a referral is made, the Department of Family Services receives it for further action.
Submit a Referral
To submit a referral for one of the System of Care (SOC) or Healthy Transitions (HT) programs, please complete the SOC and Healthy Transitions Referral Form.
