Our Unique Two-Way System
Our two-way system is unique because it allows your doctor and PreventionLink programs to talk to each other and stay updated on your progress.
A primary care doctor can easily access patient information using PreventionLink's communication system.
PreventionLink has a unique two-way system that allows your doctor and the PreventionLink programs to keep each other updated. The system tracks your progress from the moment your doctor makes a referral to a PreventionLink program through your time and attendance in the specific programs. It also tells your doctor about your progress in the PreventionLink programs. This makes it easier for you and your doctor to work together to manage and improve your health!
- Provider initiates a referral using an electronic form to PreventionLink
- PreventionLink receives the referral, processes it, and submits it to the recommended PreventionLink program
- The program receives the referral, enrolls the participant into the program, and sends progress reports to the Central Referral Service (CRS) specialist
- CRS receives the progress reports, prepares them in electronic form, and submits them to CRISP, Maryland's Health Information Exchange system
- CRISP securely shares participant health information with providers, allowing them to view participant medical history and easily coordinate care with PreventionLInk
