Assistance in Community Integration Services (ACIS)

The Assistance in Community Integration Services (ACIS) Program provides low or no barrier stable housing and support services for Medicaid enrollees at risk of institutional placement or homelessness.

Chronic Disease

The Division of Health and Wellness Services is responsible for encouraging behavioral changes that will better the health and longevity of Prince George’s County residents.

Medical Assistance

Our medical assistance programs provide a wide range of services for residents who qualify. We offer insurance programs, case management, and transportation and personal care services.

Community Health Worker Initiative

Our Community Health Workers reach out to the community and conduct assessments to connect Prince George’s County residents with the health and social services they need.
