Our Five-Year Plan
PreventionLink of Southern Maryland has a five-year plan to identify and improve the quality of life for residents at risk of or living with a chronic condition and living in underserved communities by providing easier access and affordable solutions to good healthcare.
PreventionLink Goals
- Preventing or delaying type 2 diabetes among at-risk individuals
- Helping participants get their A1C (blood sugar) levels to a healthy range to reduce their risk of heart disease and other complications such as nerve damage and vision problems

Patient Support
We support patients with:
- A strong clinical and community support system. Working with local healthcare providers and backed by a network of community health workers, PreventionLink removes known barriers to care, such as access, affordability, transportation, and financial and social support.
- Free and low-cost options. We are leading the way in healthcare by providing participants with free or low-cost options and greater access to essential healthcare services.
- Healthcare access using technology.
Programs
National Diabetes Prevention Program
The Telehealth National Diabetes Prevention Program is a forward-thinking lifestyle change program focusing on life skills, exercise, nutrition, and health education to help participants prevent diabetes. It is tailored to the unique needs of people at high risk of developing diabetes. There is an in-person and virtual version of this program.

Telehealth Diabetes Self-Management Education and Support
Following an evidence-based curriculum, the Telehealth National Diabetes Self-Management Education and Support (DSMES) program empowers participants living with Type 2 diabetes to successfully self-manage their diabetes. A certified diabetes care and education specialist and other licensed health professionals lead live, interactive classes.
Join as a Participant
People living with a chronic medical condition or are at risk for diabetes are invited to participate in the program.
