Community Health Worker Initiative

The Community Health Worker Initiative offers Prince George’s County residents free access to a Community Health Worker (CHW) who will help connect them with the health and social services they need. Through Community Health Workers, we aim to identify the needs of our residents and increase their access to quality health care.

What is a Community Health Worker?

A Community Health Worker is a frontline public health worker who works with residents to help them access the health and social services that they need. A Community Health Worker (CHW) can provide the following services to the community: 

  • Help residents enroll in health and social service programs. 
  • Help residents set and achieve personal health goals and adopt healthy behaviors. 
  • Refer residents to medical transportation and other types of assistance. 
  • Provide social support and refer clients to professional counseling. 
  • Guide residents toward Health Education, including self-management of disease screenings, prevention, and management of chronic health diseases.  
  • Coordinate resident care and case management. 

Am I Eligible? 

Any resident who is 18 years or older can apply for Community Health Worker services. To join the program, you can visit one of the locations below, or reach out via phone or email. Our number is 301-883-4707, and our email is CHWSupport@co.pg.md.us 

What Happens After I Sign Up?

Within two business days, a CHW will call you and make an assessment to determine what services you need. The CHW will help you through the process of attaining those services and follow up with you afterward to make sure that everything is going well.
