Living Well Diabetes Self-Management Program

COVID-19 UPDATE:

The health and safety of our residents are our highest priorities. With ongoing concerns about the Coronavirus (COVID-19) and recommendations from state health authorities to limit gatherings of people, we have made the decision to transition from in person programming to offering sessions online. Classes will meet virtually until COVID-19 restrictions are lifted. Once restrictions are lifted, all online sessions will transition back to in person meetings.

The Living Well Diabetes Self-Management Program is a series of self-management classes for anyone with a diabetes diagnosis. Classes meet once a week online for 2.5 hours and cover a variety of topics that support developing a healthier lifestyle and how to be a partner with your health care team.  Registration is required.

Who Should Register? 

If you have diabetes, you can take charge of your health and live well! If you wish to register for the Living Well class, please complete the online registration form. Seating is limited. Registrants should be 18 years and over. Caregivers and/or relatives are encouraged to register. Once you complete the registration form, you will receive a follow-up confirmation call or email.

Session: May 26 - July 7, 2022 (Thursdays)
Class Time: 5:00 pm-7:30 pm

Class Dates: 05/26, 06/02, 06/09, 06/16, 06/23, 06/30, 07/07
Location: Online. The link to the online program will be emailed to registered participants prior to the start of the program.
