Living Well Diabetes Self-Management Program

The Living Well Diabetes Self-Management Program is a series of self-management classes for anyone with a diabetes diagnosis. Classes meet once a week online for six consecutive weeks for 2.5 hours and cover a variety of topics that support developing a healthier lifestyle and how to be a partner with your health care team. Registration is required.

Who Should Register?

If you have diabetes, you can take charge of your health and live well! If you wish to register for the Living Well class, please complete the online registration form. Seating is limited. Registrants should be 18 years and over. Caregivers and or relatives are encouraged to register. Once you complete the registration form, you will receive a follow-up confirmation call or email.

2024 Virtual Class Schedule:

Session:  June 10 through July 15, 2024
Class Dates: 6/3 (Information Session), 6/10, 6/17, 6/24, 7/1, 7/8, 7/15
Location: Online
Time: 4:00 pm until 6:30 pm (Mondays)
