Living Well Diabetes Self-Management Program

The health and safety of our residents are our highest priorities. With ongoing concerns about the Coronavirus (COVID-19) and recommendations from state health authorities to limit gatherings of people, we have made the decision to postpone our upcoming in-person classes. As soon as we are able to reschedule our educational programming, we will be in contact with updated information.

The Living Well Diabetes Self-Management Program is a series of self-management classes for anyone with a diabetes diagnosis. Classes meet once a week for 2.5 hours and cover a variety of topics that support developing a healthier lifestyle and how to be a partner with your health care team.  Registration is required.

Who Should Register? 

If you have diabetes, pre-diabetes or diagnosed as borderline, you can take charge of your health and live well! If you wish to register for the Living Well class, please complete the online registration form. Seating is limited. Registrants should be 18 years and over.  Caregivers and/or relatives are encouraged to register. Once you complete the registration form, you will receive a follow-up confirmation call or email.
