Free Online Chronic Disease Self-Management Program

It’s the ideal time to begin to intentionally make healthier choices for our health.

A great way to do that is by signing up today for the Health Department’s latest round of FREE chronic disease self-management classes. Residents who are 18 years or older and have a chronic disease or have conditions requiring ongoing medication are encouraged to sign up. Caregivers and/or relatives are welcome to register too.

The classes meet online on Mondays for six consecutive weeks from 4:00-6:30pm. Trained instructors help participants develop healthier lifestyles and make action plans to have healthier hearts!
