2025 Community Health Resident Survey
The Prince George’s County Health Department invites anyone who lives, works, studies, plays, or worships in Prince George’s County to complete the 2025 Community Health Resident Survey. The survey is available in English, French, and Spanish and will remain open through Sunday, April 6, 2025.
Complete the Prince George’s Community Health Resident Survey
This survey is conducted by the Prince George's County Health Department in partnership with our local hospitals: Adventist Fort Washington Medical Center, Luminis Health, MedStar Southern Maryland Hospital Center, and University of Maryland Capital Region Health. Your participation will help us understand what is important to help build a healthier community. Your responses are anonymous and cannot be traced back to you. The survey should take about 5-10 minutes to complete. Thank you for taking the time to complete this survey!
